Quick Access Toolbar not working correctly

When I add any button to the quick access toolbar which is disabled with certain selections (for example the "Set Entry and Exit Point" which is only active when a segment is selected) the assigned quick access button changes to different (seemingly random) button as soon as the assigned button is disabled.
My Version:
16.0.0.7025
on a 64-bit Windows 10 machine.

Hello tom,
This issue has been fixed in the latest DG16 official version. We suggest downloading 16.1.2.7290 from PulseCloud to get the latest update, which includes a large number of bug fixes and some new features as well.
